Welcome to South Burnett Community Services. We are a registered NDIS provider. We offer a broad range of  services for people living with a disability in the South Burnett and surrounding regions to assist you to achieve your goals and meet your individual needs.

In home and community support:

We provide tailored NDIS in-home and community support to assist people with disability live more independently and stay connected to their community. This can include assistance with daily living tasks at home like help getting to appointments or social activities, support to get out in the community attending activities and events, and skills and training to build confidence for everyday independence.

Support Coordination:

Our experienced Support Coordinators can work with you to help you understand and use your plan and connect you with supports, services and therapies as identified in your goals. We can work with you to increase your capacity, live more independently, and be included in your community.

South Burnett Community Services accepts referrals from individuals, carers, family members, medical practitioners, and mental health professionals. Eligibility criteria applies.
