Graceville Centre

Welcome to the Graceville Centre at Nambour. We offer comprehensive mental health services across the Sunshine Coast region based from our office in Nambour. Recovery for individuals living with mental health concerns is about how they can gain the life skills to live their best life. We work within a recovery oriented approach that incorporates the principles of hope, personal responsibility, connectedness, discovery and developing an active sense of self. Our mental health clinicians provide care coordination and support that is recovery-focused, consulting with primary and acute care providers, family and other services to improve integration and achieve better outcomes for our participants.

Recovery focus

The Transitional Recovery Program is offered at the Graceville Centre and helps people to get back on their feet by supporting them to learn the life skills necessary to transition to independent living. The program offers up to 12 months of semi-supported, shared accommodation and up to six months of outreach support is available to people who are living in their own homes, or who have just exited the accommodation.

The Dialectical Behaviour Therapy program teaches skills around four main concepts of mindfulness, distress tolerance, emotional regulation and interpersonal effectiveness. It includes a weekly skills training group, individual counselling and a 24 hour coaching line.

The Clinical Care Coordination program ensures holistic care that addresses all aspects of the individuals mental health needs, with support that focuses on individualized plans, education and empowerment. The program operates across the Sunshine Coast South region, with referrals receive via the PHN. Self-referrals are accepted on the phone to the PHN ( 1800 595 212) and other health professionals who have the direct link to a referral form.

In-Home & Community Support

We’re here to help you imagine the possibilities and achieve your personal goals. We offer a range of in-home services such as daily living, personal care, meals, household tasks and living skills that can be tailored to your needs. We also offer community access services such as transport or accompaniment, participation, confidence and independence-building.

Transitional Recovery Program (TRP)

Recovery coaching helps people who live with a mental illness break through the barriers and build new opportunities. Psychosocial Recovery Coaching (Recovery Coach) is funded under the Support Coordination category. Recovery Coach is in your NDIS plan.

Dialectical Behaviour Therapy (DBT)

This program teaches skills around four main concepts of mindfulness, distress tolerance, emotional regulation and interpersonal effectiveness. It is partially funded by Lutheran Services and participants make a small weekly contribution. Referrals are open.

Supported Independent Living Vacancies
