Welcome to Graceville Centre at Nambour. Graceville Centre is an integral part of the healthcare community on the Sunshine Coast. We provide tailored support for people to reimagine their lives by setting and achieving their goals and learning the skills to increase their independence.

Imagine the possibilities

At Graceville Centre, we provide a range of support services and programs for people living with a disability. Graceville Centre also facilitates the Personal Development and Housing Program. The program consists of 5 share houses for 12 people. Designed to help people with life skills while they are living in our supported accommodation so they can eventually find their independence, this housing and development program supports people living with an intellectual, cognitive or psychiatric disability.

There are two Supported Independent Living properties based in Nambour. Accommodation options include a fully supported share house or semi-supported individual units close to public transport and other amenities. Clients have access to 24 hour support if needed.

For people living in their own homes, support can be accessed anywhere across the Sunshine Coast through Graceville Outreach. This service is available 24 hours a day, 7 days a week and includes in-home support and assistance with community access.

Services

In-Home & Community Support

We’re here to help you imagine the possibilities and achieve your personal goals. We offer a range of in-home services such as daily living, personal care, meals, household tasks and living skills that can be tailored to your needs. We also offer community access services such as transport or accompaniment, participation, confidence and independence-building.

Support Coordination

Support Coordinators will work with you to coordinate your services and help you to use your funding to achieve your goals. We can help you to find the best services to get the most from your NDIS plan. Support Coordination is funded under Capacity Building – Support Coordination in your NDIS plan.

Behaviour Support

Our behaviour support practitioners work with both children and adults to understand behaviours and provide positive strategies to carers and support workers. Behaviour Support is funded via Improved Relationships in your NDIS plan or is a fee for service arrangement.

Recovery Coaching

Recovery coaching helps people who live with a mental illness break through the barriers and build new opportunities. Psychosocial Recovery Coaching (Recovery Coach) is funded under the Support Coordination category. Recovery Coach is in your NDIS plan.

Dialectical Behaviour Therapy (DBT)

This program teaches skills around four main concepts of mindfulness, distress tolerance, emotional regulation and interpersonal effectiveness. It is partially funded by Lutheran Services and participants make a small weekly contribution. Referrals are open.

Killara Units (Accommodation)

Killara Units is a block of 10 fully self-contained units for people living with an intellectual, cognitive or psychiatric disability. The units are semi-supported, but residents have access to 24 hour support if needed.

Personal Development and Housing Program

The Personal Development and Housing program provides supported living accommodation for people living with a cognitive, physical and/or a psychiatric disability. The program consists of five share houses for 12 people.

Supported Independent Living (SIL)

Preparing people to live independently in the community to build their living skills. This program can help people to transition to other independent living options while receiving tailored support from people they know.
