Support for aged care residents

The Wellbeing and Positive Ageing program is fully funded by the Central Queensland, Wide Bay and Sunshine Coast Primary Health Networks. There is no cost to the provider or the resident.

Feeling sad or worried is very normal- every does. Our referrals are more for people who experience persistent low mood and persistently unsettled/worried and isolate themselves/ find it hard to cope with everyday life. Our friendly wellbeing workers provide structured one-on-one or group counselling sessions tailored to the aged care environment, for people who require emotional support or who have already an existing mental health illness.

Our holistic approach includes family and staff. All support is delivered within the residential aged care home.

The ultimate aim of the program is to improve the resident’s quality of life.

With offices in Rockhampton and Bundaberg, we are delivering the Wellbeing and Positive Ageing program to registered residential aged care facilities throughout Central Queensland and Wide Bay.

Referral form

Please note that all referrals are to be completed by a medical professional only (GP, psychiatrist, clinical nurse, registered nurse) who has the expertise to determine that symptoms are associated with mental illness and NOT with cognitive decline, dementia, and/or delirium. We cannot accept self referrals or referrals from carers.

**Residents with high suicide risk should be flagged for easy identification and prioritisation.**
