Graceville Centre at Nambour

Welcome to the Graceville Centre, Nambour

We offer comprehensive mental health services from our Graceville Centre location in Nambour on the Sunshine Coast. At Graceville clients are best served by our belief that recovery is possible for everyone.

We support people to develop their own personal understanding of recovery through exploring concepts of hope, connectedness, active self, personal responsibility and discovery.

Growth, Recovery and Client Empowerment

Our Growth, Recovery and Client Empowerment program provides long-term support to people living with severe and persistent mental illness who receive funding from the Queensland Department of Health.

We accept referrals from Queensland Health, Adult Mental Health Services.

Partners in Recovery

Partners in Recovery supports people with severe and persistent mental illness and complex needs to navigate their way around the multiple agencies involved in their care. Our team works to ensure a seamless experience for clients by facilitating coordinated responses between organisations.

Anyone can make a referral via the Partners in Recovery intake line (1300 747 724). The consent of the person being referred must be obtained prior to making the referral.

Transitional Recovery Program

This program helps people get back on their feet through supported living and life skills strategies. The Transitional Recovery Program offers up to 12 months of semi-supported, shared accommodation in three sites across Nambour and Currimundi. Up to six months of outreach support is also available to people who are living in their own homes, or who have just exited the semi-supported accommodation. Whether it’s learning about budgeting, self care, cooking skills, accessing training and employment, we can tailor the program to you.

We accept referrals from Queensland Health, Adult Mental Health Services.

Gap Initiative Program (Daliya House)

Daliya House is an integrated service that ensures people who are discharged from acute mental health care have a safe and supportive ‘home base’ from which they can secure permanent accommodation and undertake psychosocial rehabilitation.

This eight bed accommodation service is complemented by support providing practical assistance in daily living tasks, to prepare clients to move successfully back into the community.

We accept referrals from Queensland Health, Adult Mental Health Services.

Mental Health Support Program

The Mental Health Support Program helps individuals who are living with a mental illness achieve their goals. It is a fee for service program. This means that support must be privately funded or funded by a service provider you are currently engaged with.

Referrals can be made by the person requiring support, their family or a support organisation.
